INSTRUCTIONS: Please fill out the application form completely by typing your information in the correct fields.

You must submit the following documents along with your application form:

1) A clear copy of a proof-of-age document (passport or birth certificate)

2) A current photo for the printed brochure in high resolution 300dpi min.

3) Application fee via check, wire transfer, or PayPal

Application Fee for participating in both Academy & Competition: $80
Additional Forms for Academy Courses and Dorm information available via email.
Application Fee for participating in Competition only: $100
Application deadline: June 30, 2019
If the participant applies for two or more instrument categories, he/she might pay additional application fee, please email to info@lansummusicfestival.org.
Application fee non-refundable.
